Phase 1: Assessment Needs

The children made an appointment through the website, and the social worker conducted a 10-minute initial phone call to understand the situation. This included confirming that there was a domestic helper/caregiver in the household, confirming the residential estate, and confirming the patient's condition and potential for recovery.

Phase 2: Clinical Diagnosis + Providing a Solution

Physical therapist's physical assessment (30 min): Test muscle strength and balance, and record the movements that the patient is currently "unable to do".

Social worker environmental and psychological audit (15 min): Inspect for hidden dangers in the home and observe communication gaps between the domestic helper and the patient.

Explanation of the plan (15 min): Show your children the expected results and the time required for the plan.

Phase 3: Installing the rehabilitation system

Environmental improvements: For example, a shopping list for children (armrests, anti-slip pads, chair height adjustment).

Empowering domestic helpers: A physical therapist demonstrates three "core movements" and places "illustrated instruction cards" in the refrigerator.

Communication Center: Create a dedicated WhatsApp group to include children, domestic helpers, physiotherapists , and social workers.

Phase 4: Subscription Period

Daily: Domestic helpers upload practice videos daily, which are reviewed asynchronously by a professional team (responding during mealtimes).

Weekly: Perform high-intensity interventions at the patient's home as needed, and adjust the teaching difficulty of the domestic helper for the following week.

Every two weeks: SW conducts market sentiment tracking or environmental review.

Phase 5: Progress Verification

Last week of each month: Professionals call caregivers to present data (how much faster walking speed, how much lower the risk of falling, and the domestic helper's rating).
